Secondary Breast Reconstruction


Secondary breast reconstruction describes revision of a prior breast reconstruction to improve the look or feel of the breast. This can range from a small refinement to a complete reset, where a different type of reconstruction altogether is pursued. The elements of the revision are highly customized, based on a patient’s concerns and goals.

Who is a candidate?

Women who have undergone breast reconstruction and want to improve the appearance or feel of their breast (s) may be candidates for a revision operation. Whether the initial reconstruction surgery was with our Davinci surgeons or an outside surgeon, we welcome you to schedule a consultation to discuss your concerns and goals. While your original surgeon has unique knowledge of your situation, we understand that an alternate opinion can be appealing for a variety of reasons. A fresh set of eyes can prove valuable, particularly in a complicated situation. If your surgery was more recent, we ask that you schedule your consultation no sooner than six months from your most recent procedure.

What to expect

Revision breast surgery is tailored to each patients’ needs. Implant-based reconstructions can often be improved by adjusting the implant pocket (for instance, converting from under the muscle to over the muscle), changing the implant size, or softening the implant edges with fat transfer. In cases where the implant has become significantly distorted by scarring or radiation, we may offer the option of a natural tissue reconstruction, such as a DIEP. Another scenario addresses the unoperated breast in the case of a one-sided mastectomy and reconstruction. Balancing operations adjust the size or shape of the opposite breast to improve overall symmetry.

  • How soon after my original surgery can I plan a revision?

    Revisions are usually planned at least 6 months after the original surgery to ensure you are completely healed and the results are stable.

  • Can a very old reconstruction be revised?

    Yes. Breast reconstructions can change over time, creating an unfavorable shape or asymmetry between the breasts. Revisions can be performed for these reasons at any time provided you are in good health.

  • Can I schedule a consultation if my original reconstruction was done by someone else?

    Yes. Second opinions can be helpful. Please bring records from your original surgery.

  • Does revision breast surgery involve a lengthy recovery like the original reconstruction?

    While most revisions are less extensive, these surgeries are highly individualized. Dr. Davison or Dr. Pinell will review with you the recovery time that your particular surgery will require.

  • How often do implants need to be replaced?

    There is no specific timeframe for implant replacement. Implants can be replaced if there is an aesthetic concern, excessive scarring around the implant, or if the device itself has a problem, such as a rupture.

  • Can revision breast surgery be done to address pain after breast reconstruction?

    Sometimes. If we can identify the cause of your discomfort, there may be surgical maneuvers to address it. One scenario is painful animation from an under the muscle implant reconstruction. This can be revised to position the implant on top of the muscle instead.

  • Does insurance cover revisions of a prior breast reconstruction?

    Generally, yes. This is confirmed in advance of surgery.
